The City of Malden’s Health Inspection and Permitting function comes under the Malden Board of Health Office.

This application must be submitted for review and approval prior to operating a Food Service Establishment. Food Service Establishments include, but are not limited to, eat in and take out restaurants, mobile food trucks, deli’s, caterers, function halls and any establishment that prepares food items for consumption by the public. - Mobile Food Vendors Application (PDF)

This application must be submitted for review and approval prior to operating a Retail Food Service Establishment. Retail Food Service Establishments include, but are not limited to, convenience stores, quick marts, supermarkets, markets and other locations where food items are not prepared on site but are pre-packaged for retail sale - Solid Waste Hauler Application (PDF)
- State Hawkers and Peddlers License Application (PDF)
This application must be submitted to the Commonwealth of Massachusetts Division of Standards for all persons that want to operate mobile food service operations in the city. - Tanning Salon Permit Application (PDF)
- Temporary Food Service Establishment Application (PDF)
This application must be submitted for review and approval prior to running a food service that will be in operation for less than 7 days. These type of services include, but are not limited to, carnivals, parades and food festivals.
